29 CFR 1926.453(b)(2)(v):     A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:    On or about December 19, 2011, during construction activities including but not limited to carpentry work from an aerial lift, the employer did not ensure employees were protected from fall hazards by ensuring fall protection was worn when using an aerial lift.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502:    On or about 12/19/2011, during construction activities including but not limited to roofing activities, the employer did not ensure employees were protected from fall hazards by ensuring a fall protection system system/device was being used.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about 12/19/11 during construction activities including but not limited to roofing work, the employer did not ensure employees using a job made ladder to access the roof from the elevated platform was properly extended above the surface at the top with a grasping device provided.
