29 CFR 1904.41(b)(2): The OSHA Annual Survey Form was not completed and sent to OSHA within 30 calendar days, or within the time stated in the Survey From, whichever is later. Gage Food Products Company failed to complete and send to OSHA, or its agent, the "OSHA Occupational Injury and Illness Data Collection Form, 2010" within 30 days of the date Gage Food Products Company received the Form, which was mailed to Gage Food Products Company on or about July 6 2011. The Gage Food Products Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.41(b)(2), which was contained in OSHA inspection number 314031923, citation number 01, item number 001 and was affirmed as a final order on February 9 2010, with respect to a workplace located at 600 N. York Rd, Bensenville, IL 60106. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
