Lamcam, Inc. failed to complete and send to OSHA, or its agent, the "OSHA Occupational Injury and Illness Data Collection Form, 2010" within 30 days of the date Lamcam, Inc. received the Form, which was mailed to Lamcam, Inc. on or about July 6 2011. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
