29 CFR 1926.451(e)(1):When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portableladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps,walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall beused. Crossbraces shall not be used as a means of access. a. On or about November 21, 2011 - at the above addressed jobsite, employees conducting roofing operations from a ladder jack scaffold twenty two (22') feet above the ground were not provided with a proper access ladder to the scaffold platform, thereby exposing employees to the hazards associated with falls.

On or about November 21, 2011, at the above addressed jobsite, employees performing residential roofing operations on a steep sloped roof, approximately twenty three (23') feet above the ground, were not protected from falls, thereby exposing employees to fall hazards. THE ROLLER CONSTRUCTION, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313064594, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 13, 2009 WITH RESPECT TO A WORKPLACE LOCATED AT 401 LAUREL ST, WILMETTE, IL 60091.
