29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition: a. At the workplace, the Yale lift truck serial number D826N016892 (Rocky Brands #3901) had broken welds on the mount for the side rail fall protection safety system thereby exposing employees to a fall hazard.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. a. In the maintenance room, Oxygen and Acetylene gas cylinders were stored together on a cart thereby exposing employees to a fire/explosion hazard.
