Black Ridge Builders, on or about December 29, 2011 at the address of 3643 Boulder Ridge Dr. Maumee, OH: The employer did not assure that a personnel platform used to lift employees and materials to the roof was secured to the lifting carriage of the rough terrain fork lift. The employee was standing on the personnel platform while being raised up to the roof to set the truss on the front of the residential structure.

Employees were unable to identify fall hazards on the job site, exposing them to fall hazards of up to 17ft.
