29 CFR 1926.451(c)(2)(v):     Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:    a.) Black Ridge Builders, on or about December 29, 2011 at the address of 3643 Boulder Ridge Dr. Maumee, OH: The employer did not assure that a personnel platform used to lift employees and materials to the roof was secured to the lifting carriage of the rough terrain fork lift.  The employee was standing on the personnel platform while being raised up to the roof to set the truss on the front of the residential structure.

Black Ridge Builders, on or about December 29, 2011 at the address of 3643 Boulder Ridge Dr. Maumee, OH: The employer did not assure employees were protected from falls while riding on the platform to set the truss on the front side of the residential structure.  Employees were exposed to a 17 fall hazard in that no guardrails or fall protection were used while employees were lifted by the rough terrain fork lift.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) Black Ridge Builders, on or about December 29, 2011 at the address of 3643 Boulder Ridge Dr. Maumee, OH: The employer did not ensure employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems or alternative measures while performing various roofing operations. Employees were unable to identify fall hazards on the job site, exposing them to fall hazards of up to 17ft.
