29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  a) Black Ridge Builders, Perrysburg, Ohio:  On or about July 29, 2013, at the address of 23114 Pargillis Rd., Perrysburg, Ohio, the employer did not ensure that a scaffold platform used to lift employees and materials into position was attached to the forks.  Employees were raised approximately 16 feet into the air on the unsecured platform.   Black Ridge Builders was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 110757, citation number 1, item number 3a and was affirmed as a final order on April 5, 2012, with respect to a workplace located at 3643 Boulder Ridge Dr., Maumee, Ohio.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) Black Ridge Builders, Perrysburg, Ohio:  On or about July 29, 2013, at address 23114 Pargillis Rd., Perrysburg, Ohio, the employer did not ensure that employees were protected from falls while riding on and working from an elevated scaffold platform. Employees who were raised into position by a rough terrain forklift were exposed to falls of approximately 16 feet to the ground below, as no form of fall protection was being used.  Black Ridge Builders was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 110757, citation number 1, item number 3b and was affirmed as a final order on April 5, 2012, with respect to a workplace located at 3643 Boulder Ridge Dr., Maumee, Ohio.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) Black Ridge Builders, Perrysburg, Ohio:  On or about July 29, 2013, at a worksite located at 23114 Pargillis Rd., Perrysburg, Ohio, the employer did not ensure that each employee had been trained in the recognition and control of fall hazards.  Employees exposed to falls of approximately 16 feet were not trained to use fall protection systems, which could result in permanent disability or death.    Black Ridge Builders was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 110757, citation number 1, item number 4b and was affirmed as a final order on April 5, 2012, with respect to a workplace located at 3643 Boulder Ridge Dr., Maumee, Ohio.
