A. Black Ridge Builders worksite located at 14919 Stonebridge Lane, Perrysburg, Ohio: On or about March 7, 2016, the employer did not ensure the ladder used to access the working level was extended three feet above the landing surface. An 8 foot step ladder was being used exposing the employees to falls while using the ladder to access the uppermost level.

Black Ridge Builders worksite located at 14919 Stonebridge Lane, Perrysburg, Ohio: On or about March 7, 2016, the employer failed to ensure the top of a 6ft A-framed stepladder was not use as a step by employees. The employees were using the top of the ladder to access the ridge being installed. The employees were exposed to fall hazards due to improper ladder use.

A. The employer does not require the use of any form of fall protection during framing activities. The violation was most recently observed at 14919 Stonebridge Lane, Perrysburg, Ohio, where employees were not protected from a 18.00 foot fall to the ground below. To abate this hazard the employer must ensure that its employees performing framing activities are protected from falls, and no employees are allowed onto a structure more than 6 feet high without adequate fall protection.    Black Ridge Builders was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 which was contained in OSHA inspection number 110757, citation number 1, item number 4a and was affirmed as a final order on March 15, 2012, with respect to a workplace located at Maumee, Ohio.
