29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:      (a)	Bulk tank ladder to Production roof: On 12/29/2011 and at times prior thereto, an intermediate ladder landing at 9 6 high was not protected to prevent a worker from walking directly into the opening.    (b)	 Bulk tank ladder to Production roof: On 12/29/2011 and at times prior thereto, the ladder opening to the top of the tank at 16 high was not protected to prevent a worker from walking directly into the opening. (b)	Production: On 12/29/2011 and at times prior thereto, workers accessed a 9 3 high platform that was not protected by standard railing to service a vitamin E tote. (b)	Load Out Pump 2: On 12/29/2011 and at times prior thereto, an electrical control box in a wet location (outdoors) was mounted directly to a concrete curb with inadequate airspace between the box and mounting surface.
