29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a.) Precision Machine & Tool Company located at 1016 N. 5th St. Fremont, OH; On or about January 4, 2012, the employer did not assure an energy control program consisting of employee training, periodic inspections and providing lock out devices, was established to prevent caught by and amputation hazards while changing the blade on the Powermatic Horizontal Band Saw.

Precision Machine and Tool Company located at 1016 N. 5th St. Fremont, OH; On or about January 4, 2012, the employer did not assure employees were protected from caught by and amputation hazards while using the Bridgeport Milling Machines with unguarded chucks to drill holes into metal parts.    b.) Precision Machine and Tool Company located at 1016 N. 5th St. Fremont, OH; On or about January 4, 2012, the employer did not assure employees were protected from amputation hazards from the unused portions of the blade, while using the Powermatic horizontal band saw to cut metal pieces to size.

Precision Machine & Tool Company located at 1016 N. 5th Street Fremont, OH 43420; On or about January 4, 2012, the employer did not assure that the right side of the Black and Decker pedestal grinder was equipped with a work rest that was properly adjusted to 1/8th inch.  Employees that operated the grinder were exposed to injuries from potential jammed work and/or breakage of the wheel.    b.) Precision Machine & Tool Company located at 1016 N. 5th Street Fremont, OH 43420; On or about January 4, 2012, the employer did not assure that the both sides of the DoALL pedestal grinder was equipped with a work rest that was properly adjusted to 1/8th inch.  Employees that operated the grinder were exposed to injuries from potential jammed work and/or breakage of the wheel.
