29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) Colonial Cleaners, Inc. located on 5031 Lewis Avenue Toledo, OH: On or about January 5, 2012, the employer did not assure employees who were potentially exposed to burn hazards while using an ACE Industrial Electric Iron were protected by appropriate personal protective equipment, such as, but not limited to hand protection or sleeve protectors.  Employees were exposed to temperatures that ranged from 146 to 300 degrees Fahrenheit.     b) Colonial Cleaners, Inc. located on 5031 Lewis Avenue Toledo, OH: On or about January 5, 2012, the employer did not ensure that a protective shield or barrier such as, but not limited to, machine cover was used in the press area.  Employees are exposed to thermal hazards while working in close proximity to the Cissell Sleeve Finisher which reaches temperatures near 276 degrees Fahrenheit.

29 CFR 1910.147(c)(1): The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      a) Colonial Cleaners, Inc. located on 5031 Lewis Avenue Toledo, OH: On or about January 5, 2012,  the employer did not have a lock out program for the control of the hazardous energy, which includes machine specific procedures, training, and periodic inspections, while performing maintenance and/or servicing machines such as but not limited to, the Unipress machine.  Failure to lock out these components while these tasks are being performed exposes the employer to caught in and shock hazards.

29 CFR 1910.305(b)(1): Conductors entering boxes, cabinets, or fittings were not protected from abrasions:      a) Colonial Cleaners, Inc. located on 5031 Lewis Avenue Toledo, OH: On or about January 5, 2012, the employer did not ensure that conductors entering the steam pressure relief valve on the backside of the ACE Industrial Electric Iron (flathead iron)  located in the press area was protected from abrasion and was effectively closed.  Employees were exposed to 120V electrical shock hazards.
