On or about March 3, 2009, an employee had an injury to the right ankle but there was no description of the object/substance that directly injured or made the person ill.    c.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2009 a description of work-related injuries and illnesses.  On or about June 3, 2009, an employee had an injury to the right shoulder but there was no description of the object/substance that directly injured or made the person ill.    d. On or about  September 9, 2009, an employee had an injury to the right knee but there was no description of the object/substance that directly injured or made the person ill.    e.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2009 a description of work-related injuries and illnesses.  On or about September 12, 2009, an employee had an injury to the right quadriceps but there was no description of the object/substance that directly injured or made the person ill.    f.
