29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year:    The employer did not complete a log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) for 2009 in the detail as required by the regulation.    a.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2009 a description of work-related injuries and illnesses.  On or about February 1, 2009, an employee had an injury to the left hand/wrist but there was no description of the object/substance that directly injured or made the person ill.    b.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2009 a description of work-related injuries and illnesses.  On or about March 3, 2009, an employee had an injury to the right ankle but there was no description of the object/substance that directly injured or made the person ill.    c.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2009 a description of work-related injuries and illnesses.  On or about June 3, 2009, an employee had an injury to the right shoulder but there was no description of the object/substance that directly injured or made the person ill.    d.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2009 a description of work-related injuries and illnesses.  On or about  September 9, 2009, an employee had an injury to the right knee but there was no description of the object/substance that directly injured or made the person ill.    e.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2009 a description of work-related injuries and illnesses.  On or about September 12, 2009, an employee had an injury to the right quadriceps but there was no description of the object/substance that directly injured or made the person ill.    f.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2009 a description of work-related injuries and illnesses.  On or about October 1, 2009, an employee had an injury to the right shoulder but there was no description of the object/substance that directly injured or made the person ill.    g.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2009 a description of work-related injuries and illnesses.  On or about November 9, 2009, an employee had an injury to the right ankle but there was no description of the object/substance that directly injured or made the person ill.      Abatement certification required within ten days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

On or about July 7, 2010, an employee had an injury to an arm but there was no description of which arm or of the object/substance that directly injured or made the person ill.    d.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2010 a description of work-related injuries and illnesses.  On or about July 27, 2010, an employee had a back sprain/strain but there was no description of the object/substance that directly injured or made the person ill.    e.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2010 a description of work-related injuries and illnesses.  On or about September 29, 2010, an employee had an arm injury but there was no description of which arm or of the object/substance that directly injured or made the person ill.     f.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2010 a description of work-related injuries and illnesses.  On or about November 2, 2010, an employee had a laceration to the forehead but there was no description of the object/substance that directly injured or made the person ill.    g.  At the facility located at 4531 Columbus Road, Centerburg, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2010 a description of work-related injuries and illnesses.  On or about November 7, 2010, an employee had an injury to the left side but there was no description of the specific body part injured or of the object/substance that directly injured or made the person ill.      Abatement certification required within ten days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

On or about  August 22, 2011, an employee had an injury to the ribcage but there was no description which side of the rib cage or of the object/substance that directly injured or made the person ill.
