29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to travel with the load trailing whenever the load obstructed forward view: Employees working near the fork truck were exposed to struck-by hazards. The fork truck operator was carrying a stack of wooden pallets that obstructed the driver's forward view and he did not drive with the load trailing.

29 CFR 1910.213(i)(1): Non-working portion(s) of the blade of bandsaw(s) were not enclosed or guarded: Employees operating the portable pallet band saw were exposed to amputation hazards. The band saw blade was not guarded or marked in any way to prevent employees from placing their hands within one foot or less from the cutting edge of the blade. Employees did not use a push-stick to advance the pallet into the saw blade.
