29 CFR 1926.451(b)(1)(ii):     Where the employer makes the demonstration provided for in paragraph (b)(1)(i) of this section,the platform shall be planked or decked as fully as possible and the remaining open space between the platform and the uprights shall not exceed 9 1/2inches (24.1 cm).Exception to paragraph (b)(1): The requirement in paragraph (b)(1) to provide full planking or decking does not apply to platforms usedsolely as walkways or solely by employees performing scaffold erection or dismantling. In these situations, only the planking that the employerestablishes is necessary to provide safe working conditions is required.      a)  At the site, an employee was working from one wooden plank on a tubular welded frame scaffold that was two bucks high with each buck measuring at 7 feet in height.  The scaffold was not properly planked with only one plank installed, which was on the second buck measuring 10 feet 11-1/2 inches above the ground.

29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part.      a)  On the site, an employee was working on a tubular welded frame scaffold that was over ten feet high without being protected by a fall hazard by means of a guardrail system, fall arrest system, or safety net.

29 CFR 1926.452(c)(2):     Frames and panels shall be braced by cross, horizontal, or diagonal braces, or combination thereof,which secure vertical members together laterally. The cross braces shall be of such length as will automatically square and align vertical members sothat the erected scaffold is always plumb, level, and square. All brace connections shall be secured.    a)  At the site, an employee was observed working on the second buck of a tubular welded frame scaffold with each buck measuring 7 feet in height.  The scaffold did not have the required amount of cross bracing attached to the frame.  It was observed that the scaffolding only had two cross braces installed, when four is required with a scaffold that is two bucks high.

29 CFR 1926.451(e)(1):     When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portableladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps,walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall beused. Crossbraces shall not be used as a means of access.    a)  At the site, an employee was working from a tubular welded frame scaffold.  The scaffold was erected two bucks high with each buck measuring seven feet in height.  The employee would climb the scaffold's frame in order to gain access to the scaffold's platform which measured 10' 11" from the ground.  The scaffold lacked a safe means of access whether by ladder, tower, walkway, or other acceptable means.
