29 CFR 1926.350(a)(9): Compressed gas cylinders shall be secured in an upright position at all times except, if necessary,for short periods of time while cylinders are actually being hoisted or carried. a. On the first floor where welding work was performed, the employer left a cylinder of oxygen and a cylinder of acetalene unsecured and not placed on a cart thereby exposing employees to a struck-by hazard.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface shall be protected from tripping in or stepping intoor through holes (including skylights) by covers. a. On the lower level of the casino construction site, the employer removed dirt and concrete from a hole to repair a penetration from the sink and did not cover or place protection around the hole thereby exposing employees to a trip/fall hazard.
