29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.      Employees were engaged in a residential re-roofing and re-sheeting operation in Dayton, Ohio and exposed to a fall hazard in excess of 19 feet while not utilizing any type of fall protection.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      Employees were utilizing a Werner 32 foot aluminum extension ladder to access the roof while being engaged in a residential roofing operation and the ladder did not extend 3 foot above the landing surface.
