29 CFR 1926.1053(b)(6):     Unsecured ladder(s) were used on surfaces that were not stable and level:    Employees installing shingles were exposed to a fall hazard of approximately 27 feet to a lower level, due to the fact that the ladder used to access the roof was placed on a sloped roof and was not secured.

29 CFR 1926.1060(a):     The employer did not provide a training program for each employee using ladders and stairways, that enabled each employee to recognize hazards related to ladders and stairways, and that trained each employee in the procedures to be followed to minimize those hazards:    Employees were exposed to a fall hazard of approximately 27 feet using a ladder to access the roof from the roof of the house next door. The ladder was unsecured and not on a level surface and the employer did not provide training for each employee that would have enabled them to recognize the hazards and provided them with the procedures to follow to minimize these hazards.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems.    On January 6, 2012, employees were exposed to fall hazards of up to approximately 27 feet while performing roofing activities without appropriate fall protection in place.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee who might be exposed to fall hazards which enables each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize the hazards.    Employees were exposed to fall hazards while performing roofing activities without appropriate fall protection in place and were not trained to recognize the hazards of falling and the procedures to be followed in order to minimize the hazards.
