CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights was no more than 1 inch (2.5 cm) wide:    	(a)	Centennial Plastering, 2321 Champa, Denver, CO 80205:  On and before January 6, 2012, the exposing employer of eight employees working on a buildings face off tubular scaffolding did not install planking so that the space between adjacent units and the space between the platform and the uprights was no more than 1 inch wide.  Scaffolding working surface was not fully planked exposing employees to fall hazards of 12 feet while working off the scaffold working surface.

CFR 1926.451(e)(1):  When scaffold platforms where more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used;       (a)   Centennial Plastering, 2321 Champa, Denver, CO 80204: On and before January 6, 2012, an employee working on a 12 foot tall tubular scaffolding was observed accessing the working surface of the scaffold system without an adequate ladder as described above.  Condition exposed employee to a fall hazard of approximately 12 feet when accessing the work area.

(a)	Centennial Plastering, 2321 Champa, Denver, CO 80205:  On and before January 6, 2011, the exposing employer did not ensure that employees performing stucco and plastering work off the working surface on a 12 foot high tubular scaffold were protected from falling by an adequate fall protection system. Condition exposed employees working off the scaffold to fall hazards of approximately 12 feet.

CFR 1926.451(f) (3):  The employer did not provide for inspections for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.      	(a)	Centennial Plastering, 2321 Champa, Denver, CO 80205:  On and before January 6, 2012, as the exposing employer of eight laborers working on a buildings face off of tubular scaffolding, the employer did not provide for an adequate number of inspections of the scaffolding system.  Employees were observed working without adequate fall protective measures in place, due to a lack of adequate job site scaffolding inspections by the controlling employers competent person. Condition exposed employees to fall hazards of over 12 feet.

CFR 1926.95(d)(1):  Protective equipment, including personal protective equipment (PPE), used to comply with this part, was not provided by the employer at no cost to employees.    	(a)	Centennial Plastering, 2321 Champa, Denver, CO 80205:  On and before January 6, 2012 the employer did not provide personal protective equipment at no cost to his employees at prescribed by OSHA regulations. This lack of an adequate personal protective equipment exposed employees to head and eye hazards.
