29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) There was no program for hazardous energy control (lockout) with training for employees who perform any servicing or maintenance of equipment, such as, but not limited to, saw blade changes.

29 CFR 1910.147(d)(3):     All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source:  (a) On or about January 9, 2012, the saw blade was changed on the Peterson panel saw and the main power was not de-energized as only the stop button was activated.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, etc.:  (a)  The Peterson panel saw was inadequately guarded in that there was no lower blade guard to prevent accidental contact with the blade and inadequate machine guarding for the blade travel near the operator's station as the operator reaches around and holds small parts that the hold downs will not clamp and reach the point of operation with one hand when the other hand was operating the hydraulic travel lever.
