29 CFR 1926.451(g)(1):     Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    On or about 01/06/12 during roofing activities, the employer did not ensure employees used fall protection systems/devices when using a ladder jack scaffold at a height of 25 feet.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    On or about 01/06/12 during roofing activities, the employer did not ensure employees used fall protection systems/devices when working on a roof with a 12/12 pitch and at a height in excess of 25 feet.      Note: The employer was not following the rescinded residential fall directive STD 03-00-001 or the general residential fall standard concerning fall protection.

29 CFR 1926.452(k)(1):     Platforms exceeded a height of 20 feet (6.1 m):    On or about 01/06/12 during roofing activities, the employer did not ensure employees exceeded the 20 foot maximum platform height when using a ladder jack scaffold.   The height of the scaffold platform was 25 feet.
