29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    A. Williams Roofing worksite located at 4820 Lakeside Dr., Monclova, OH: On or about January 9, 2012, the employer did not ensure employees wore proper eye protection while operating pneumatic nail guns to secure asphalt shingles. Employees were exposed to potential flying object hazards while operating the pneumatic fasteners on the roof of the two story residential structure.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    A. Williams Roofing worksite located at 4820 Lakeside Dr., Monclova, OH: On or about January 9, 2012, the employer did not ensure employees were protected from falls while installing weatherproofing on the two story residential structure with a roof pitch of 9/12.  The employee was exposed to a potential fall distance of approximately 20.00 feet.

29 CFR 1926.1053 (b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.     A. Williams Roofing worksite located at 4820 Lakeside Dr., Monclova, OH: On or about January 9, 2012, the employer did not ensure the side rails of the aluminum extension ladder was extended at least three feet above the landing surface located directly above the garage door. The ladder was being used to access and egress the roof to perform weatherproofing.
