29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: a) On or about January 6, 2012, and at times prior, the employer did not allow for adequate clearance for electrical panel MDP - 19,21,23. Panel was found in direct proximity of a walkway and was less than 30 inches from a spray booth enclosure.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: a) On or about January 6, 2012, and at times prior, employees were exposed to respiratory hazards during spray painting and sanding operations from wearing a half face filtering facepiece respirator with facial hair that interfered with the seal on the facepiece.
