29 CFR 1926.350(a)(10):     Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials(especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistancerating of at least one-half hour.    a.    At the front of the site where work was being performed, the employer stored an oxegon cylinder with an acetalene cylinder thereby exposing employees to a fire/explosion hazard.

29 CFR 1926.404(f)(6):     Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent andcontinuous.      a.    In the Chiller area of the plant where work was being performed, the extension cords used to supply power to the portable tools were not equipped with ground pins thereby exposing employees to a contact hazard.

29 CFR 1926.405(b)(1):     Conductors entering boxes, cabinets, or fittings. Conductors entering boxes, cabinets, or fittingsshall be protected from abrasion, and openings through which conductors enter shall be effectively closed. Unused openings in cabinets, boxes, andfittings shall also be effectively closed.       A.    In the general contractors trailor, the circuit breaker box used to turn on and off the lighting each day had openings in the energized panel box thereby exposing employees to a contact hazard.

29 CFR 1926.453(b)(2)(v):     A body belt shall be worn and a lanyard attached to the boom or basket when working from anaerial lift.Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable aspart of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under1926.502(e).    a.    At the front of the site where an employee operated the aerial lift, the employer did not ensure employees were wearing fall protection thereby exposing employees to a fall hazard.
