29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): a) Employees were not protected from the hazard of cave-in while working in a trench measuring approximately 5.7 feet in depth: Xpert Sewer Service, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA violation number 315156760, citation 1, item 1, issued on April 21, 2011, and became final order on or about May 11, 2011, with respect to a workplace located at 8646 S. Kimbark Ave., Chicago, IL 60619. Xpert Sewer Service, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA violation number 313088866, citation 1, item 5, issued on May 11, 2009 and became final order on or about May 27, 2009 with respect to a workplace located at 2290 N. Cedar, Des Plaines, IL 60018. No abatement certification or documentation is required for this item.
