Facility - On or about January 5, 2012, employees were required to operate the Jet lathe with an unguarded chuck. The employees were thereby exposed to the hazard of rotating/moving parts.

29 CFR 1910.213(b)(3): On applications where injury to the operator might result if motors were to restart after power failures, provision shall be made to prevent machines from automatically restarting upon restoration of power.    a. Facility - On or about January 5, 2010, employees were required to use a Craftsman radial arm saw.  The radial arm saw did not have a magnetic motor starter.  Employees were thereby exposed to hazards associated with drive motor engagement after power failures.

29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed.  The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.    a.  Entire facility - On or about January 5, 2011, the employer had not adequately certified periodic inspections of the established energy control procedures in place for employees who perform servicing and maintenance of equipment.
