29 CFR 1910.147(c)(1):  Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine orequipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall beisolated from the energy source and rendered inoperative:    (a)  There was no program on energy control procedures, employee training, and periodic inspecitons to include providing locks and training to employees who performed setups on the CNC mills and lathes, cleaned and set up the Pines Benders, and changed the blades on the Marvel band saws.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operatorand other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips andsparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc:      (a)  Employees operating the Central Machinery Vertical Band Saw, were exposed to 5 inches of unguarded blade.    (b)  Employees operating the Pines Bender (S/N 106-50-56-2672) were exposed to pinch points created by the opening and closing clamps and collets, as well as being exposed to the moving carriage.    (c)  The guarding provided on the Pines Bender (S/N 11010-2932 ) was inadequate, in that the safety mat placed in front of the tube bender was not anchored in place and was paritially pushed under the tube bender, thereby allowing the operator to load and unload the tube bender without actually standing on the safety mat.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.215(a)(4):  Work rests. On offhand grinding machines, work rests shall be used to support the work. They shallbe of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with amaximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work restshall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion:    (a)  The tool rest on the Dayton Pedestal Grinder was not adjusted to within 1/8th inch of the grinding wheel, in that the gap between the grinding wheel and the tool rest on the left side of the grinder was 1-1/4 inches.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.215(b)(9):  Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of thisparagraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to theconstantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs(b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of theperipheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.):    (a)  The upper peripheral guard on the Dayton Pedestal Grinder was not adjusted within 1/4 inch of the grinding wheel, in that the left side of the grinder had a gap of 1 inch between the guard and the abrasive wheel.
