29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: a) K & D Masonry LLC - On or about January 12, 2012, at address 91 Hedges Street, Tiffin, OH, the employer failed to ensure that employees were protected from falls while working from a tubular welded frame scaffold. Employees were assisting with the grout bucket from the scaffold platform without an acceptable form of fall protection, exposing them to fall of approximately 26 feet to the ground below.
