29 CFR 1926.451(e)(4)(ii): The toprail of each stairrail system shall also be capable of serving as a handrail, unless a separate handrail is provided. At the worksite, the employer did not ensure that a handrail was provided for employees accessing/egressing roof from stairway tower scaffolding, thereby exposing employees to a potential trip/fall hazard.

29 CFR 1926.502(f)(1)(i): When mechanical equipment is not being used, the warning line shall be erected not less than 6 feet (1.8 m) from the roof edge. At the northwest corner of the worksite, the employer did not ensure that the warning was erected at least six feet from the roof edge, thereby exposing employees to an approximate 30 foot fall hazard.
