Located at Kinetics Noise Control, Incorporated Dublin, Ohio: On or about 01/10/2012, the employer did not record the following workplace injuries and illnesses correctly on the OSHA 300 log for the calendar year 2009:    a) On or about April 6, 2009, an employee sustained an injury to their right hand (Case 1) and column "F" of the OSHA-300 for 2009 did not include all the information required. Column "F" must include a description of the injury, the parts of the body affected, and the object/substance that directly injured or made employee ill.    b) On or about March 31, 2009, an employee sustained an injury to their back (Case 2) and column "F" of the OSHA-300 for 2009 did not include all the information required. Column "F" must include a description of the injury, the parts of the body affected, and the object/substance that directly injured or made employee ill.    c) On or about June 1, 2009, an employee sustained an injury to their left hand (Case 3) and column "F" of the OSHA-300 for 2009 did not include all the information required.
