29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service, or were placed in service if the examination showed conditions adversely affecting the safety of the vehicle: a) Diamond Manufacturing of Bluffton, located on 505 East Jefferson Street in Bluffton, OH: On or about January 12, 2012, the employer did not assure that defects that could potentially affect the safety of the forklift were reported and corrected immediately. The forklift trucks tires were damaged and it had a hydraulic leak that potentially exposed an employee to stability and struck by hazards.

Extreme care shall be exercised; the block shall be "inched" into the limit or run in at slow speed. 29 CFR 1910.252(b)(2)(iii): Protection from arc welding rays. Where the work permits, the welder should be enclosed in an individual booth painted with a finish of low reflectivity such as zinc oxide (an important factor for absorbing ultraviolet radiations) and lamp black, or shall be enclosed with noncombustible screens similarly painted. Booths and screens shall permit circulation of air at floor level. Workers or other persons adjacent to the welding areas shall be protected from the rays by noncombustible or flameproof screens or shields or shall be required to wear appropriate goggles: a) Diamond Manufacturing of Bluffton, located on 505 East Jefferson Street in Bluffton, OH: On or about January 12, 2012, the employer did not ensure that the welding area in the facility was enclosed with noncombustible or flameproof screens or shields, exposing employees, adjacent to the welding area, to eye injuries from the welding rays.
