29 CFR 1910.106(e)(9)(iii): Combustible waste material and residues in a building or unit operating area were not kept to a minimum, stored in covered metal receptacles and disposed of daily: a. Tire and Lube Center- Employees were exposed to fire hazards where rags used to absorb combustible fluids were stored in an open top plastic bucket marked "Rags" and were not disposed of on a daily basis.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a. Tire and Lube Center- Employees retrieving tires from the tire racks were exposed to overhead struck by hazards when pulling tires down from the rack using a metal pole hook that allowed for the tires to drop to the floor. The employer did not ensure that head protection was used or did not disallow the hazardous work practice.
