One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.      On or about January 12, 2012 and at time prior thereto, employees were exposed to an amputation hazard when the safety interlock was bypassed on the HandTMan sausage stuffer.

Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed so as to prevent moisture or water from entering and accumulating within the enclosures and shall be mounted so there is at least 6.35-mm (0.25-in.) airspace between the enclosure and the wall or other supporting surface. However, nonmetallic enclosures may be installed without the airspace on a concrete, masonry, tile, or similar surface. The enclosures were not weatherproof in wet locations.      On or about January 12, 2012 and at times prior thereto, employees were exposed to a shock while using outlets which are located in wet and damp locations such as but not limited to the sausage room and the packaging room.
