29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300 for the year 2010: a) On or about January 18, 2012, W.W. Grainger, Inc. did not provide injury descriptions for 4 recordable injuries on the 2010 OSHA Form 300. On June 9, 2010, June 24, 2010, June 28, 2010 and August 30, 2010, recordable injuries occurred, 2 of which resulted in either job transfer or restriction. Column F did not describe the injury, parts of the body affected and the object/substance that caused the injuries. b) On or about January 18, 2012, W.W. Grainger, Inc. did not provide complete descriptions for 13 recordable injuries on the 2010 OSHA Form 300. Recordable injuries occurred in 2010 on August 12, August 30, September 13, October 17, October 29, November 1, November 8, November 12, November 17, November 23, December 6, December 22 and December 27. Column F did not describe the object/substance that caused the injuries. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
