29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could not walk directly into the opening: (a) Platform ladderway openings between jumbo vessels 101 to 108 did not have the open passage way guarded with a swing gate or offset, exposing employees to a seven (7) foot fall hazard.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) No procedure was developed for pigging processes performed on but not limited to loadout vessel piping, conche vessel piping and jumbo vessel piping; to ensure that hazardous pneumatic pressure was isolated prior to installing pigging device and prior to removing device from termination point.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) The attempted retreival of the pipe pigging device from the termination catch point resulted in the violent release of hazardous pneumatic energy, in-that the residual pipe air pressure was not verified to be isolated to zero (0 psi), exposing the employee(s) to a struck by hazard.
