29 CFR 1910.28(j)(4): The workman was not protected by a safety life belt attached to a life line. (a) Blanchard Valley Farmers Cooperative, Inc. 318 N. High Street, Mount Blanchard, Ohio: On or about October 25, 2011, the employer did not ensure that the employee, suspended approximately 60 feet above the ground in a boatswain's chair, was protected by a life line. The employee working from the suspended chair, which was attached to the crane's hook, wore a full body harness that was also connected to the crane's hook but was not secured by a life line. The employee was at risk of serious injury from falling if the hook failed or if the chair and harness straps slipped from the hook.
