29 CFR 1910.146(c)(4):   When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146:  (a)  The employer did not develop and implement written procedures for entry into permit spaces to include items such as but not limited to:  written policy guidelines and permit system, potential hazards evaluation and entry preparation, potential hazards evaluation and entry preparation, emergency procedures, personnel training, and maintenance activities and programs review for spaces such as but not limited to:  dust collection system.

29 CFR 1910.212(a)(3)(ii):   Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  (a)  In the Recraft area, an Adler sewing machine had a distance between the needle and the shoe was approximately 3/8" exposing employees to struck-by hazards.  (g)  In the Recraft area, a WJ Young Co. heeler pressed the nails from the top into the shoe.  A ram came down exposing employees to struck by hazards.  (h)  In the Handsewn area, the Pamco (Make:  American, Model:  UHN-A, S/N:  913) heeler would drive nails into the soles of the shoe.  The point of operation was not properly guarded exposing employees to struck by hazards.  (i)  In the Handsewn area, the Pamco (Make:  United, Model:  UET-M, S/N:  236) trimming machine had an unguarded trimmer with twelve blades which rotated and trimmed the excess sole from the shoe exposing employees to struck by hazards.  (j)  In the Handsewn area, the FHR trimming machine had a wider unguarded blade which rotated and trimmed the heel exposing employees to struck by hazards.  (k)  In the Recraft area, the Hudson Machinery (#00233) trimmed the excess heel base with an unguarded trimmer exposing employees to struck by hazards.  (l)  In the Recraft area, the S-850 Stanley had unguarded/inadequately guarded trimmers which would trim the edge of the shoe and a groove in the shoe exposing employees to struck-by hazards.  (m)  In the Recraft area, the United Lever (#001344) had a two-hand trip which activated two rollers which would smooth the soles on the shoes exposing employees to caught-in hazards.

29 CFR 1910.217(c)(2)(i)(a):   Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s):  (a) In the Woodlore area; the Work Center (J.S. Richardson) had a router with a 4" wide plexiglass guard and a dual saw blade with a 6" wide plexiglass guard.  The guards were easily removable and employees could reach around it exposing them to struck-by hazards.
