29 CFR 1910.147(d):     The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    (a) In the Grinding Area,  Machine Operator(s) performed set up without first locking out the G006 External Grinder (Mitsubishi Model RD23-B35A, Serial No. GK511).  This condition exposed the worker to the potential hazards associated with unexpected start up of the machine.      Or, in the alternative:      29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) In the Grinding Area, a method of machine guarding, such as interlocks, was not provided to protect Machine Operator(s) of the G006 External Grinder (Mitsubishi Model RD23-B35A, Serial No. GK511) from the hazard of rotating parts.

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a) The employer's written hazard communication program was deficient in that it did not include a description of the NFPA labeling system being used in the plant.  Hazardous chemicals used in the plant included, for example, Ferrocote 364 BF, a combustible liquid used for washing parts, which could cause irritation to eyes and skin, and could be harmful if inhaled.

29 CFR 1910.1200(f)(5):     The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein:  (a) In the Bristol Cell area, adjacent to the G008 Grinder, a parts washer which contained Ferrocote 364 BF was not labeled with its identity and an appropriate hazard warning.  This substance was a combustible liquid.  Effects of exposure to this substance could include irritation to skin, eyes, and respiratory system.

29 CFR 1910.1200(h)(1):     Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    (a) Hazard communication training appeared to be deficient in that some workers had no recollection of having been provided hazard communication training and did not know the location where material safety data sheets were kept in the work site.
