29 CFR 1926.451(b)(2): Each scaffold platform and walkway was not at least 18 inches (46 cm) wide. (a) Gase Enterprises, LLC on or about January 19, 2012, at the address of 1200 N. Main St. Bowling Green, OH: The employer failed to ensure the working platform supported by welded frame scaffolding which employees were working, was at least 18in wide. An employee performing framing work on a commercial structure was exposed to a potential 8ft fall to the ground below while working from a 12in wide aluminum pick.

Main St. Bowling Green, OH: The employer did not ensure employees were protected from falls while installing new roofing material to a steep sloped roof on a commercial structure. Employees were exposed to falls of up to 14ft to the ground below.
