29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation: a. Entire Facility - On or about January 23, 2012, all required information for column "F" on the OSHA 300 log forms was missing. The incomplete year's OSHA 300 log forms were the following; 1. Case numbers 1,2,13, 19 and 21 for calendar year 2010 were missing information such as the object/substance that directly injured or made person ill. 2. Case number 20 for calendar year 2010 was missing the entire information on column "F". 3. Case numbers 1, 2, 7, 8 and 12 for calendar year 2011 were missing information such as the object/substance that directly injured or made the person ill. 4. Case numbers 3, 4, 9, 10, 11 and 13 for calendar year 2011 were missing the entire information on column "F". 5.Case numbers 1 and 2 for calendar year 2012 were missing information such as the object/substance that directly injured or made the person ill.
