29 CFR 1910.147(c)(4)(ii):  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules,and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    (a)  The machine specific energy isolating procedures were inadequate for the CNC machines, in that they did not include a specific statement on the intended use of the procedures, specific procedural steps for shutting down, isolating, blocking, securing equipment and for the removal and transfer of lockout devices and the responsibility for them, as well as specific requirements for testing machines to verify the effectiveness of lockout devices.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operatorand other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips andsparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.:    (a)  The guards on the CNC mills, including the Fadal #1 VMC-40 Vertical Machining (Milling) Center, were not interlocked with the machine control system.     (b)  The guards on the Okuma MC-4VA CNC Mill were not interlocked with the machine control system, and the guards did not enclose the machine, thereby exposing the operators head and shoulders to potential injuries should the parts or tooling shatter and/or fly out.         In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
