29 CFR 1910.24(b):     "Where fixed stairs are required." Fixed stairs were not provided for access from one structure level to another where operations necessitate regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations. Fixed stairs shall also be provided where access to elevations is daily or at each shift for such purposes as gauging,inspection, regular maintenance, etc., where such work may expose employees to acids, caustics, gases, or other harmful substances, or for which purposes the carrying of tools or equipment by hand is normally required.     The two sets of portable steps used by employees to access the (laser) plasma table located in the high bay area of the facility, were not affixed to the work platform.

1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.       On or before 01/05/2012, the employer did not ensure that various equipment or machinery used at the site was effectively guarded.  The unguarded equipment includes:      a.    The (8 inch) unused portion of the blade on the Kalamazoo horizontal band saw, located in the forge shop;      b.     The chuck of the Guardian Power drill press, located in the forge shop;            c.      3 to 4 Roll Benders, mfg. include but are not limited to; the Lown roll bender located in the forge and the Berth &           Company roll bender located in the low bay;     d.     The (approx. 3 1/2 inch to 4 inch) unused portion of the blade on the vertical band saws located in the low bay            area of the shop;     e.      The Giddings & Lewis Fraser boring mill and the Cintimatic boring mill, both located in the low bay area of the            facility;                f.      The 3 Monarch automatic  lathes, located in the low bay area of the facility, didnot have the chuck guarding           installed.     g.     The point of operation of the Cincinnati-shear (with a gap in the guarding which measured approximately 1/2 inch)           and the pneumatic hold-downs (which measured approximately 1/2 inch);      h.     The plasma (laser) cutting machinery exposed employees to point of operation hazards and flying sparks.
