29 CFR 1910.36(d)(1):     Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:      a) On or about 19 January 2012, the buildings west-wing emergency exit door was locked and the door handle was missing.  Employee(s) working on the lower level (basement) of the facility were not able to open the exit route door from the inside, at all times, limiting the ability to escape a fire on the lower level.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.157(c)(1):     Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    On or about 19 January 2012, the employer did not ensure that portable fire extinguishers were identified and mounted such that they were readily accessible to employee(s) during an emergency:    a) On the ground floor of the shop area, access to a fire extinguisher was blocked by Power Steering Fluid Exchanger unit.  In addition, there were no signs pointing out the location of the fire extinguisher.    b) On the lower level of the shop area, access to a fire extinguisher was blocked by a basket with used rags.  In addition, there were no signs pointing out the location of the fire extinguisher.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a) On or about 19 January 2012, the employer did not develop or implement a written hazard communication program in accordance with 29 CFR 1910.1200(e)(1) that would describe or include at least the following:    1) Requirement for labeling of containers of hazardous chemicals;  2) Material safety data sheet availability;  3) Training of employees;  4) A complete list of hazardous chemicals known to be in the workplace;  5) Methods to inform employees of the hazards on non-routine tasks; and  6) Methods to inform other employer(s) of material safety data sheet availability; the labeling system; and any precautionary measures to protect employees.    Automotive technician(s) changing automobile fluids (engine, transmission oils) were exposed to hazardous chemicals including but not limited to: Valvoline Max Life Motor Oil (contains heavy paraffinic distillates; alkylated phenol; zinc alkyldithiophosphate); Valvoline Dex/Merc ATF (contains heavy paraffinic distillates); Car Brite Hot Stuff Concentrated Cleaner/Degreaser pH e 13 (contains ethylene glycol monobutyl ether; sodium hydroxide; sodium xylene sulfonate; sodium tripolyphosphate).    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employee(s) working on the lower level (basement) of the facility were, limited in their ability to escape a fire on the lower level.
