29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) The written lockout program and procedures were inadequate in that they did not include tasks, such as, but not limited to, cleaning of the glue machine with exposure to accidental start-up and being cuaght in the nip points of the rollers as requiring lockout.  (b) There were no documented procedures for equipment with multiple energy sources, such as, but not limited to, the Bruno presses, the Relco press and the baler to ensure the isolation of all energy sources.  (c)  The written program stated that tags alone could be used for lockout even though the machinery would accept locks.

29 CFR 1910.147(c)(7)(i):     The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  (a)  The glue machine operator and other operators were not trained as authorized employees to perform lockout during tasks, such as, but not limited to,  cleaning operations, where exposed to accidental start up or activation of roller nip points on the glue machine.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts, etc.:  (a) On or about January 14, 2012, the Black Brothers gluing machine (s/n 181130) was inadequately guarded as the hinged guard on the feed side was not secured in position and/or interlocked with the operating controls to stop the glue rolls in the event the guard is lifted out of position exposing the ingoing nip point of the top rolls.
