Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification.      a)  Worksite, Interior - Employees were using an outlet box that was hanging from the ceiling to power a circular saw and a radio; on or about 01/18/12.          Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.405(b)(2):   Covers and canopies. All pull boxes, junction boxes, and fittings were not provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth,well-rounded surfaces on which the cords may bear.    a)  Worksite, Interior - Employees were using an outlet to power a radio and a circular saw.  The outlet did not have a cover; on or about 01/18/12.      Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
