29 CFR 1910.147(c)(1):     Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine orequipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    The Company had not developed an energy control program consisting of energy control procedures, employee training and periodic inspections.

29 CFR 1910.178(l)(1)(i):     The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    Operators of the Jungheinrich Pallet Truck had not received training and an evaluation specified in paragraph (l) to determine their competency to operator the pallet truck.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points.     In the Production Area an operator was exposed to the inrunning nip points of the rollers located on the front and back of the first set of rollers while operating a Sakari 272 EPI Printing Press Machine.

29 CFR 1910.305(g)(1)(iv):     Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:    In the Production Area next to the Ryobi 3300 printing press machine,  a green extension cord set, 110 Volt AC, was being used to power a radio and run under a rubber mat and subject to damage.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    In the Production Area, on the Toscon portable air compresssor used with the Sakari 258 EPI Printing Press Machine, the fitting on the 110 Volt AC wiring was pulled away from the electrical box creating strain on the terminal screws.

29 CFR 1910.178(l)(4)(iii):     An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.    The Company had not conducted an evaluation of operator's of the sit-down Nissan counterbalanced forklift at least once every three years.
