29 CFR 1926.501(b)(14):     Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:      a)  Employees were walking/working on a second floor surface within three feet of door wall openings which had outside bottom edges greater than 9.5 feet above lower levels and inside bottom edges less than 5 inches above the walking/working surface without utilizing any fall protection equipment.    b)  Employees were walking/working on a second floor surface within three feet of window wall openings which had outside bottom edges greater than 9.5 feet above lower levels and inside bottom edges less than 30 inches above the walking/working surface without utilizing any fall protection equipment. 29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:    a) An employee was working from a top plate of a wall at a height greater than six (6) feet above the second floor surface and greater than fifteen feet above a concrete lower level surface without utilizing any fall protection equipment.    b)  Employees were walking/working on a second floor surface near staged materials with the surface edge greater than 9.5 feet above compacted surface without utilizing any fall protection equipment.    c)  Employees were accessing work areas by walking along a second floor edge at a height greater than 9.5 feet above concrete surface which was between the areas without utilizing fall protection equipment.    THE BRUCE BOEYINGA DBA WOODWISE CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 98340, CITATION NUMBER 01, ITEM 001, AND WAS AFFIRMED AS A FINAL ORDER ON 11/02/11, WITH RESPECT TO A WORKPLACE LOCATED AT 1600 CORNFLOWER, PEORIA, IL  61615.
