Worker(s) were exposed to potential fall hazards, from a height of 13.6 feet, in that the working level of the scaffold system was not fully planked

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.        Worker(s) were observed engaged in commercial construction activities installing stucco material on the exteriors sides of an  existing structure. Worker(s) were exposed to potential fall hazards from a height of 13.6 feet in that a proper means of access and egress was not provided.

29 CFR 1926.451(f)(3):     Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.      Workers were engaged in commercial construction activities renovating four exterior sides on an existing structure. Workers were exposed to potential fall hazards from heights of 13.6 feet in that the scaffolding system was not properly inspected prior to the start of work.

Worker(s) were observed engaged in commercial construction activities renovating the exterior four (4) sides of a existing structure. Workers were exposed potential fall hazards from a height of 13.6 feet in that a proper fall protection system was not utilized.

Worker(s) were observed engaged in commercial construction activities renovating the exterior four (4) sides of a existing structure. Workers who were exposed potential fall hazards from a height of 13.6 feet were not properly trained in the recoginition and  elimination of scaffolding  hazards.
