29 CFR 1910.133(a)(2):    The employer did not ensure that each affected employee used eye protection that provides side protection when there was a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.    On or about March 27, 2012, an employee in the Small Wheel Area was exposed to eye injuries from surface cleaning in the area with compressed air and the employer failed to ensure the employees safety glasses were equipped with side protection.      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    Employees were exposed to crush and amputation hazards from unexpected energization of the Mattison Grinding Machine (Model #:24, SN: 24624) while performing maintenance operations, and the employer failed to effectively develop and implement written procedures for the lockout tagout operations in accordance with this standard in the following instances:    a. On or about March 27, 2012, a Big Wheel Finishing Department employee was replacing the grinding pads on the Mattison Gringin Machine by entering and sitting in the machine for disassembly placement and tightening of the grinding pads without employing specific lockout tagout procedures.    b. On or about March 6, 2012, employees from the Maintenance Department replaced a brass nut and screw by disassembling a back panel cover that guarded chain and sprocket systems in the machine without employing specific lockout tagout procedures.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(f)(3)(i):     A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group:    On or about March 6, 2012, employees from the Maintenance Department were exposed to amputation hazards from unexpected energization of the Mattison Grinding Machine (Model #:24, SN: 24624) while replacing a brass nut and screw replacement by disassembling a back panel cover that guarded chain and sprocket systems in the machine and the employees used one lock to which all maintenance employees had master keys for.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
