29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail systems, warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a) Rossford Hotels, LLC dba Country Inn & Suites Toledo South, Rossford, Ohio:  On or about January 23, 2012, at address 9790 Clark Drive, Rossford, Ohio, the employer did not ensure that employees were protected from falls repairing roofing material (shingles) to a low sloped roof (4:12) on a commercial type structure.  Employees were exposed to potential falls of approximately 28 feet.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards:    a) Rossford Hotels, LLC dba Country Inn & Suites Toledo South, Rossford, Ohio:  On or about January 23, 2012, at address 9790 Clark Drive, Rossford, Ohio, the employer did not ensure that employees had been trained in the appropriate use of fall protection, such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing repair.  Employees were exposed to falls of approximately 28 feet to the ground below, which could result in permanent disability or death while fixing a shingle that had come loose.

OSHA forms 300 and/or 300A were not maintained for calendar year 2012.  The company had approximately 19 employees and had 1 recordable injury.
