29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18 inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end:    a. On or about January 25, 2012, at the above addressed jobsite, employees working from the ladder jack scaffold platform were exposed to falls as one end of the platform extended past the support approximately three feet.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps,walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used:    a. On or about January 25, 2012, at the above addressed jobsite, employees accessing and egressing the ladder jack scaffold platform were exposed to falls due to the fact that the platform was supported on the outside of the ladders which required the employees to crawl over the platform to get on or off the platform, rather than stepping on or off the platform from an access ladder.

On or about January 25, 2012, at the above addressed jobsite, employees working from the ladder jack scaffold were exposed to falls of approximately 14 feet 10 inches due to the lack of personal fall arrest systems.
